In order for us to best serve you, we recommend that you contact your insurance company directly (see the toll-free number on the back of your card). When you call, make sure to tell the agent you are requesting benefit information for "Rehabilitative Therapies or specifically Physical Therapy and/or Occupational Therapy/and or Speech Therapy"as it applies to you.

The key information you need is:

• What is my co-pay or co-insurance? (This is the amount you have to pay each visit.)
• What is my deductible amount? (This is your annual out-of-pocket expense for medical care before the insurance company will pay anything)
• How much of my deductible have I met? (Until this is met, you will be responsible for full payment at time of treatment.)

You may also go to your insurance plans website and search for the specific provider type around your area (input zip code or city).  You will find a list of the IN NETWORK PROVIDERS that are close to your area.

Please contact our office directly to find out if we are providers under your insurance plan.  We will also submit billing in your behalf.  Often as IN NETWORK PROVIDERS your out of pocket costs are the least.  Some deductibles are waived but not all.  Co-pays and Co-insurances are set and your out-of-pocket costs are the least as compared to an out of network provider.
